CONSENT AND TREATMENT INFORMATION
This page provides important information about the services you will receive at HerHerbs.
Missed / Late and Cancelled Appointments
To achieve the best outcomes in your treatment, our Naturopaths thoroughly review your questionnaire answers and conduct a 20-minute telehealth consultation. We thus value our patient's time and our own, so if you know you are going to be late for an appointment, please reschedule via your confirmation email or reminder message that is sent before your appointment. If you miss your consultation, please reschedule ASAP for continuity and results.

Side effects and adverse reactions
As with all medicines, side effects can occur when taking a new product. In some circumstances, minor side effects can occur, including bowel changes, nausea and headaches. These are common and will be discussed in your consultation. Please continue taking your herbal medicine sinus and thus if for any reason you experience an unwanted side effect, please email your Naturopath ASAP and cease taking your herbal tonic. If in the unlikely event a severe reaction occurs, please call the poisons hotline on 13 11 26.
Confidentiality and Privacy
Your treatment with HerHerbs is confidential and private. This means we will not disclose the information you provide to us to third parties unless you consent for us to do so. It is important to note that there are limits to confidentiality in exceptional circumstances including when there is a risk to you or others, or if we are responding to a subpoena or warrant. You can discuss this with your practitioner at any point and will answer any questions you might have.
Health Record Management
We will create and keep a health record for you which will record all relevant information relating to the treatment provided to you.
We may also collect and store health information about you from third parties such as your doctor. Your health records are managed and protected in accordance with the applicable state and federal laws. Health records are legally required to be stored for a number of years following the conclusion of your treatment with us. You are also able to request access to a copy of your health record.
Please discuss this with your practitioner should you wish to do this at any time.
Nature of Services Provided
Her Herbs practitioners are fully qualified Naturopaths, and registered with a professional association. We use solely herbal medicines and provide educational resources on dietary and lifestyle interventions that will support your health. At the end of each consultation, your practitioner will ensure you understand the rationale for the approach, identified any risks, and ensure you are happy to commence treatment.
Limitation to Services
Her Herbs is a private provider of Naturopathic services. Treatment is provided via pre arranged, scheduled telehealth consultations. Practitioners have limited availability for contact outside of scheduled appointments and thus may not have an opportunity to respond to between-session contact prior to your next appointment, so this should not be relied upon for emergency matters. HerHerbs practitioners will only provide services that are within their professional capabilities, competence and role as a private provider of Naturopathic services. If treatment needs are identified that are outside of this scope, your practitioner will endeavour to direct you to more appropriate services.

Her Herbs is not an emergency service. We are not able to respond to urgent or emergency matters and our communication channels (Phone, SMS, Email) are not always monitored outside of business hours. Young persons, children and minors
Her Herbs is aimed for women of reproductive age, including those experiencing menopause. Therefore, we unfortunately do not offer our services to those under 12 years of age, or who have not experience at least 2 menstrual cycles.
Consent

You may withdraw this consent at any time in the future however please note that some components are irrevocable; for example, we are legally required to keep a copy of your health record even if you withdraw consent and cease treatment.
In order to attain a comprehensive assessment and best treatment outcomes, we require your consent to collect information about you; ask questions of a personal nature; and ensure you maintain consistency with your herbal formulas, consultation and at home activities including dietary and lifestyle changes.
